Application forms

Here you will find the forms you need if you wish to apply for leave from studies, non-completion of studies, parallel studies or to change from temporary identity to a Swedish personal identity number.

Application for approved leave from studies

If you have been admitted to studies at the undergraduate or graduate level and wish to take a break in your studies with the aim of resuming them at another time, you can apply for an approved leave from studies. Once granted an approved leave from studies, this means that you are guaranteed a place to the course for a specified semester if you sign up for the resumption of studies in accordance with the rules set out in the Rules for study administration at first- and second-cycle levels that you will find at this page.

If you are not granted an approved leave from studies, you can read more in the rule concerning the possibility of notifying a non-completion of studies with the announced intention of resuming studies within a year.

The application for an approved leave from studies is submitted to the course or programme coordinating department, or equivalent. Contact the study advisor (or equivalent) at the course or programme coordinating department for more information.

Notification for non-completion of studies

If you notify non-completion of studies, this means that your admission to the course or programme in question will cease to apply and that you are not guaranteed a place if you wish to resume your studies at a later course or programme time.

Before you complete and submit the notification, it is important that you read the rules for the non-completion of studies in the Rules for study administration at first- and second-cycle levels in order to, among other factors, gain information on how a notification for the non-completion of studies with the announced intention to resume studies within a year affects your possible resumption of studies. In the rules, you will also find information on what you need to do in order to resume your studies. The rules are available in the Legal framework for Umeå University.

The notification for a non-completion of studies is submitted to the course or programme coordinating department/equivalent.

Application for parallel studies

If you want to study more than 45 credits per term at Umeå University you need to apply for parallel studies.

Only candidates who have accumulated 60 credits will be granted parallel studies. The Appendix is sent to Student Services/Admissions Office with the application or as an addition to a previously submitted application to Umeå University.Application for parallel studies

Application for a change from temporary identity to Swedish personal identity number (personnummer) at Umeå University

If you are not registered in the Swedish Population Register when admitted to studies, you will automatically be given a temporary identity in the admissions system and in the study documentation system (Ladok). The studies you start and complete will be registered in Ladok under your temporary identity.

If you later on get registered in the Swedish Population Register, you must apply for a change from your temporary identity to the Swedish personal identity number in order to get your completed studies correctly registered in Ladok. Fill in this form and send it by internal mail to Student Services/Student Records Office, or hand it in to Infocenter.
